is there an "other" section for you to describe the dispute? otherwise speak with the bank again and ask for their advice on what is best to fill out. The form provided seems very rigid in what you can and can't dispute.

The other part of the form requires mrrchant details and the last part is for my declaration. There is no option whatsoever for me to furnish "other" reasons/information not adressed by their form. It seemed they ignored the contents of the letter i sent with the form to explain my case.
